Palliative and Transitional Care
Palliative and Transitional care services include the Regional Palliative Care Program (RPCP), Hospice, Outpatient Palliative Care Clinic and Transitional care for those waiting for an alternate destination such as Long-term Care or Supportive Housing.
Access to high quality palliative care can have a profound impact on the quality of life of both the individual who is dying and their loved ones. The North West Local Health Integration Network (LHIN) RPCP goal is to mobilize, strengthen, and reorient the health care system to improve access to safe, comprehensive, and high quality palliative care for all residents of Northwestern Ontario.
